Several clinical markers are used to classify HCC and select treatments; these include serum alpha fetoprotein (AFP) level, Barcelona Clinic Liver Cancer (BCLC) stage, and TNM stage. However, these markers are generally poor at predicting prognosis after surgery, since they may not reflect chronic inflammation that can promote tumor cell proliferation and help tumors evade immune surveillance. 4 The markers also do not reflect malnutrition, which has been associated with worse prognosis of HCC patients. 5 , 6 Most HCC patients are in a severe inflammatory state due to chronic viral infection, imbalance between pro- and anti-inflammatory immune cells, and injuries or hematologic alterations as a result of surgical resection.
